University Health Services, Florida State University's fully accredited primary care facility, provides medical care and outreach programs through a team of dedicated professionals. As a student-focused, student-friendly center, its mission is to advance the quality of life for university students,improving academic performance and increasing retention. University Health Services works in conjunction with many university departments, but is part of the Division of Student Affairs to maintain our focus and attention on serving students.

Please Note: There is a new emergency center on 1260 Metropolitan Blvd near I10. Be aware that if you have the FSU Florida Blue insurance it will be billed as an emergency room visit and will be out of network should you use this center. We recommend going to this center ONLY in a life-threatening emergency.

In every other case, it would be less expensive to go to the Urgent Care Center on 1541 Medical Drive next to the Tallahassee Memorial Hospital or to any Patients First.

Health Compliance Checklist

  1. Health History Form
  • You can
  1. Fax them to 850-644-8958,     OR 
  2. Mail them to Health Compliance Office, 960 Learning Way, Tallahassee, FL 32306-4178,      OR
  3. Use the electronic drop box to email it.  Instructions are in the packet.      OR
  4. Bring them to the Health Compliance Office, University Health Services.   Forms submitted at Orientation may take up to 5 days to process.

We bill all insurances. We are currently contracted providers with Aetna, Blue Cross Blue Shield, CIGNA, Humana and United. We are working on becoming contracted providers with CHP and Tri-Care.

Welcome orientation parents and students. Please remember to fax in your completed health history form and complete your online insurance waiver/purchase prior to your arrival at FSU. This will help prevent any delays in registering for classes. PLEASE DO NOT FAX OR MAIL COPIES OF YOUR INSURANCE CARD.

The student sponsored insurance does not cover participation in collegiate athletics. Check with Nick Pappas at 850-645-2700 for insurance options. Participation in intramural sports and sports clubs is covered.  The student health plan does cover FSU Cheerleader and Golden Girl members as sports club participants.

WELCOME  to the University Health Services’s Student Insurance Management Information System (SIMIS). 

Healthy students meet their academic goals.  University Health Services administers the requirement for insurance coverage for the university.  Every full-time student new to the university must show proof of comparable health insurance prior to registration once each year or purchase the student health insurance.

Other insurance is acceptable as comparable if it meets minimum requirements listed under Private Insurance Requirements to the left.

APPOINTMENTS

University Health Services is an appointment based wellness center.

Appointments are scheduled by calling the Central Appointment Unit at   850-644-4567.  Appointments are required for all clinics including the General Medical Clinics, Psychiatric Clinic, Women’s Clinic, and Allergy Clinic.

If you are experiencing life threatening problems, call 911!!

Please remember to bring your FSU-Card and your insurance card with you to your scheduled appointment and show them to the receptionist when you check in.

Please Arrive On Time!  Patients should arrive at least 15 minutes prior to routine appointments. Some appointments will require arrival 1 hour prior to the scheduled time. Please refer to appointment reminder information or inquire in the clinic if you are unsure. 

If you are unable to make your scheduled appointment, please call 24 hours in advance to cancel.
